Your Opportunity
Every young person deserves the chance to feel connected, supported, and hopeful about their future. As a Case Manager within our Reconnect team in Mandurah, you will work alongside young people aged 12 to 18 and their families who may be experiencing family conflict, disengagement from education or community, housing instability, or risk of homelessness. Through trusted relationships, practical support, informal counselling, advocacy, and coordinated case management, you will help young people navigate challenges and strengthen their connections to family, education, employment, and community.
Working as part of a small and dedicated team, this is an on-site position where you will provide intensive, person centred support tailored to each young person's unique goals and circumstances. From conducting assessments and developing support plans to facilitating family engagement and connecting young people with services, you will play a vital role in creating meaningful outcomes that help build safety, stability, and long term wellbeing.
Your Key Responsibilities:
· Respond to referrals from young people, families, schools, community organisations, and government agencies, conducting intake, assessment, and risk assessment processes.
· Build strong therapeutic relationships with young people and families to identify immediate needs and support long term goals.
· Deliver intensive case management, including support planning, case reviews, progress monitoring, and accurate case documentation.
· Provide informal counselling, advocacy, and practical support to assist young people experiencing family conflict, housing instability, disengagement, or other complex challenges.
· Facilitate family engagement and family reconciliation activities, including working collaboratively with parents and caregivers to strengthen relationships and outcomes.
· Connect clients with internal and external services that support health, education, employment, housing, recreation, and social participation outcomes.
· Develop and maintain strong relationships with schools, community organisations, and local stakeholders while promoting the Reconnect program through networking and community engagement activities.
Requirements for Success
· Degree or Diploma in Social Work, Social Welfare, Social Science, Community Services, or a related discipline, or substantial equivalent experience in a similar role.
· Demonstrated experience supporting vulnerable, disadvantaged, or at risk young people, particularly those experiencing family conflict, trauma, disengagement, or housing instability.
· Strong understanding of the challenges impacting young people and families, including complex trauma, family breakdown, substance use, and social isolation.
· Proven ability to engage, motivate, and build rapport with young people while delivering person centred and strengths based support.
· Well developed knowledge of community, government, and nongovernment services, with experience making effective referrals and coordinating support networks.
· Strong organisational, administration, and time management skills, with the ability to manage competing priorities, maintain accurate records, and oversee an active caseload.
· Ability to work autonomously while contributing to a collaborative team environment, demonstrating resilience, sound judgement, and an understanding of the impacts of vicarious trauma and self care practices.
· Full driver licence with no restrictions.
Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
