About the role
Love people? Good at solving problems? Thrive when there’s always something to figure out? We should talk.
At Commonwise Home Care, our mission is simple: To Truly Care for People. We provide in-home care that helps older adults live safely and comfortably in their own homes, and we believe the people providing that care deserve to be cared for, too.
We're looking for a Care Coordinator to join our team. This is a people-focused, fast-paced role that brings together scheduling, operations, customer service, relationship management, and problem-solving. You'll work alongside a dedicated team to coordinate care for our clients and support the caregivers who make that care possible.
We sometimes describe care coordination as a combination of air traffic control and matchmaking. You need the right person, in the right place, at the right time. But it's about more than filling a shift. It's understanding what matters to a client, getting to know our caregivers, anticipating challenges, and making thoughtful decisions that help everyone succeed.
It's rewarding work, but it isn't always easy. Plans change. People get sick. Schedules fall apart at the worst possible time. Families need reassurance, caregivers need support, and someone has to figure out what happens next.
That someone could be you.
You don't need previous home care experience to be successful here. If you've worked in sales, operations, hospitality, retail management, education, customer service, or another role where you've had to manage competing priorities and build relationships with all kinds of people, you may have exactly the experience we're looking for.
WHAT YOU'LL DO
Coordinate Care and Manage Scheduling
• Coordinate caregiver schedules for clients receiving in-home care, ensuring shifts are covered by qualified caregivers who are a good fit for each client's needs.
• Manage daily home care scheduling, including new service requests, schedule changes, caregiver availability, call-outs, and last-minute coverage needs.
• Match caregivers with clients by considering availability, experience, care needs, personality, preferences, and continuity of care.
• Communicate with clients, family members, and caregivers by phone, email, and text to confirm schedules, answer questions, and resolve concerns.
• Keep client and caregiver schedules, records, notes, and other information accurate and up to date in our scheduling software.
• Look ahead, identify potential scheduling gaps, and work proactively to prevent problems before they affect a client's care.
Solve Problems and Keep Things Moving
• Respond to unexpected changes with urgency, sound judgment, and a solutions-oriented mindset.
• Work through the occasional scheduling scramble when a caregiver calls out or a client needs a change in coverage.
• Balance competing priorities while recognizing which situations require immediate attention and which can wait.
• Take ownership of problems rather than passing them along. You'll have a team to lean on, but you'll also need to think independently and follow through.
• Help improve scheduling processes and identify ways to make care coordination more efficient and effective.
Build Relationships and Deliver Great Service
• Get to know our caregivers and clients as people, not just names on a schedule.
• Listen carefully, ask good questions, and understand what clients and caregivers need to be successful.
• Build trust through consistent communication, follow-through, and a willingness to have honest, sometimes difficult conversations.
• Provide professional, compassionate support when caregivers need coaching or clients and family members are frustrated.
• Partner with our recruiting, clinical, and care management teams to make sure the right people and information come together at the right time.
• Help create a positive experience for clients, families, caregivers, and your teammates.
Handle the Details
• Maintain accurate scheduling records, client notes, caregiver information, and required documentation.
• Learn and use our scheduling and care management software, including WellSky.
• Answer incoming calls, respond to messages, follow up on outstanding items, and complete the administrative work that keeps the operation running smoothly.
• Support the team during particularly busy periods, including occasional evening or weekend coverage as needed.
WHO WE'RE LOOKING FOR
We care more about how you think, how you work, and how you treat people than whether you've held this exact job before.
You might be a great fit if you:
• Are a natural problem-solver. When something goes wrong, you want to figure out why and what to do next.
• Know how to work with people. You can build rapport, listen well, communicate clearly, and adapt your approach to different personalities.
• Are exceptionally organized. You can keep track of details, deadlines, conversations, and multiple moving pieces without losing sight of the big picture.
• Can prioritize under pressure. You know how to stay focused when several things need your attention at once.
• Take initiative. You don't wait to be told about every next step. You notice what needs to happen and take responsibility for getting it done.
• Follow through. You understand that a missed call, an incomplete note, or an unconfirmed shift can have real consequences for another person.
• Have a service mindset. You care about giving people a good experience, even when the conversation is difficult or the answer isn't what they hoped to hear.
• Can be both compassionate and direct. You're comfortable showing empathy while holding people accountable to expectations.
• Enjoy being part of a team. You collaborate, share information, ask for help when needed, and step in when a teammate needs support.
• Bring positive energy and a sense of humor. We take our work seriously, but we don't believe every day has to feel serious.
This role is especially well suited to people who have experience in sales, operations, scheduling, hospitality, customer service, education, retail management, dispatch, or other coordination-heavy roles. Those backgrounds can build many of the skills that make an excellent Care Coordinator.
QUALIFICATIONS
• Strong written and verbal communication skills.
• Comfort using computers, learning new software, and managing information across multiple systems.
• Excellent organizational skills and attention to detail.
• Ability to work independently, exercise sound judgment, and manage competing priorities.
• Willingness to learn the home care industry, understand client care needs, and follow established processes.
• Flexibility to help support scheduling needs during occasional evenings, weekends, holidays, or other high-demand periods, as required by the team's coverage plan.
