You’ll use your full nursing skill set, build meaningful relationships, and help patients safely recover and remain independent in their own homes throughout Bryan-College Station.

- Completing comprehensive initial and ongoing patient assessments
- Developing individualized plans of care based on patient needs and physician orders
- Providing skilled nursing visits in patients’ homes
- Managing an assigned caseload and coordinating services across the care team
- Monitoring patients’ physical, psychosocial, environmental, and caregiver needs
- Identifying changes in condition and communicating appropriate interventions
- Educating patients and caregivers on medications, diagnoses, treatments, and disease management
- Collaborating with physicians, therapists, social workers, and clinical leaders
- Updating care plans as patients’ needs and goals change
- Completing accurate and timely clinical documentation

- Current Registered Nurse license in the state of practice.
- Graduation from an accredited nursing program.
- At least one year of nursing experience in a Home Care setting (we want to set you up for success!).
- Home health experience and familiarity with Homecare Homebase (HCHB).
- Valid driver’s license with an acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including assisting with patient transfers and moving equipment.
