Seeking full time Care Manager are to promote whole person, integrated care by planning, coordinating, tracking, closing of gaps in care and monitoring care to individuals to achieve the key goals of the behavioral health tailored plans. The Care Manager shall report to the Care Manager Supervisor.

Requirements

Minimum of Bachelors degree in Human services or related field.

2 years experience working with children/adolescence in the mental health field.
