Intake Coordinator N

Conduct initial intakes with veterans and /or family members screening for eligibility for the organization's suicide prevention program. Coordinate case assignment with 'Ohana Resiliency Center multi-disciplinary team. Document detailed intakes on the electronic health records (EHR) system; maintain comprehensive client referral tracking system. Prepare reports as requested by Clinical Supervisor. • Provide services via telehealth platform, telephone, or in person. • Meet regularly with Clinical Supervisor to coordinate case assignments, client activities at satellite locations, report referral to admission conversion rate; Provide ongoing program evaluation and recommendations to the Clinical Supervisor for continuous growth and quality. • Be active in and network with veteran community groups. Provide presentations at various community groups within the greater metropolitan and surrounding area. Promote the success and reputation of the 'Ohana Resiliency Center and its programs. • Knowledge of local community resources and provide warm hand-offs to case manager for veterans and families needing external and internal referrals. • Manages all intake calls and appointments and administers appropriate paperwork, psychometric screeners, and scheduling with appropriate program staff for continued care. • Any additional duties as assigned

Monday-Friday 8 am to 4:30 pm with some evening and weekend hours.
